Job summary

About the Role:

As an Adult Social Worker in this role, you will work with adults aged 18+ across various needs, including physical disabilities, learning disabilities, mental health conditions, and older adults. The role involves conducting strengths-based assessments, developing care plans, and delivering interventions tailored to individual circumstances. You'll work closely with individuals, families, carers, and partner agencies to ensure safe, effective, and person-centered support.

Key Responsibilities:

• Undertake Care Act assessments, reviews, and support planning in line with our client 3 conversational model
• Complete Mental Capacity Assessments and Best Interest decisions as required
• Manage safeguarding enquiries in accordance with relevant legislation and procedures
• Work with adults with physical disabilities, learning disabilities, mental health needs, and older people
• Conduct community-based assessments
• Develop collaborative multi-agency working relationships
• Maintain accurate case recordings and reports using the Mosaic Database system
• Ensure compliance with safeguarding legislation and best practices

Candidate Responsibilities:

• Hold a recognised Social Work qualification
• Be registered with Social Work England
• Have at least 2 years of post-qualification experience as a Social Worker
• Demonstrate a sound knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation
• Possess excellent assessment, communication, and report-writing skills
• Manage a complex caseload independently and efficiently
