Mildmay Mission Hospital is seeking a Substance Misuse Worker to support inpatients on HIV, Homeless and Substance Misuse recovery programmes. Qualifikationen

• Experience in care planning and working with substance misuse services.
• Experience managing a caseload and working with vulnerable adults.
• Experience in a multidisciplinary team and facilitating group sessions.
• Knowledge of housing/resettlement and welfare benefits is beneficial.

Aufgaben

• Provide person-centred support and psychosocial interventions for adults.
• Assess patients, develop care plans and manage risk in relation to substance misuse.
• Support discharge planning and liaise with internal and external services.
• Promote harm reduction and empower patients in recovery.
• Contribute to MDT meetings and complete relevant reports and referrals.
