A leading community healthcare provider in Bristol seeks motivated Inpatient Rehabilitation Nurses to deliver high standards of care for frailty and stroke patients. Join a multidisciplinary team focused on improving patient independence and quality of life. This role offers opportunities for both newly qualified and experienced nurses to develop their skills in a supportive environment.
We are looking for motivated and compassionate nurses who have a genuine interest in working with frailty and stroke patients to deliver high standards of care. We are looking for nurses who are wanting to provide our patients with the best experience and treatment possible and are ready for the challenge of helping us to further develop our rehabilitation services.
The rehab units are based within South Bristol Community Hospital. We have 2 wards a frailty unit and a sub-acute stroke unit. (SSARU)
If you are a Registered Nurse who enjoys working with patients to improve quality of life and promote independence then this is an exciting opportunity to join us in our rehabilitation units.
You will be an integral multidisciplinary team, all working together, to deliver individualised nursing and rehabilitation care to patients with the common goal of delivering the best outcomes possible for our patients.
In both units patients present with a broad range of conditions including stroke, orthopaedic, frail elderly, post-surgery and other long term neurological conditions and we work with them to help them attain the highest level of personal independence prior to discharge.

The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for frailty and stroke patients and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
The post holder will-
Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the stroke and frail cohort.
Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.
