Skylark Rehabilitation Unit is a 30-bedded facility in Yate (South Gloucestershire), providing support and active rehabilitation for patients on Discharge to Assess Pathway 2. You will work as part of our multidisciplinary team, delivering individualised nursing and rehabilitation care to patients with conditions such as orthopaedic issues, frailty, post-surgery recovery, and neurological conditions, aiming for the highest level of personal independence prior to discharge.
The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of service users, ensuring high standards of patient-focused care that prioritise safety, dignity, and respect.
