Job description

We are looking for motivated nurses who have a genuine interest in working with elderly service users and delivering high standards of care. Skylark Rehabilitation Unit is a 30-bedded facility in Yate (South Gloucestershire), providing support and active rehabilitation for patients on Discharge to Assess Pathway 2. You will work as part of our multidisciplinary team, delivering individualised nursing and rehabilitation care to patients with conditions such as orthopaedic issues, frailty, post-surgery recovery, and neurological conditions, aiming for the highest level of personal independence prior to discharge.

The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of service users, ensuring high standards of patient-focused care that prioritise safety, dignity, and respect.

Key Responsibilities:
  1. Provide, monitor, and ensure a high standard of safe and effective nursing care through professional actions, verbal, and written communication.
  2. Under the support of senior staff, conduct initial holistic assessments, implement, and evaluate care programmes for acutely and chronically unwell patients, often with complex needs from the frail elderly cohort.
  3. Perform routine and non-routine tasks including medication administration, clinical observations, care planning, assisting with daily care needs, health promotion, venipuncture, and discharge planning.

Additional Information:
  • You will work weekends and bank holidays on a rostered basis, with day and night shifts primarily during the day shift (07:00 - 19:30) and night shift (19:00 - 07:30).
