Job summary

Bedfordshire Hospitals NHS Foundation Trust is recruiting an experienced Nurse to join the Diabetes Multidisciplinary Team at Bedford Hospital. The post targets Band 6 nurses with acute medicine exposure or demonstrated management experience, focusing on inpatient diabetes care in the North Bedfordshire Diabetes Centre.

The role involves delivering education, developing individualised care plans, improving discharge pathways, and sharing diabetes knowledge with the MDT, patients, and families.

Qualifications

• Recent Band 6 experience in acute medicine.
• (or evidence of management)
• Experience in inpatient diabetes care preferred.
• Ability to assess, plan and implement care pathways.
• Strong communication with MDT and families.

Responsibilities

• Assess, plan and implement ongoing evaluated pathways of care and education for people with diabetes and their families and carers.
• Plan and implement timely and safe discharge from hospital, liaising with community diabetes staff as needed.
• Provide advice and information to people with diabetes and their carers to improve understanding and self-management.
• Discuss treatment issues and develop individualised care plans under the Diabetes Inpatient Specialist Nurse guidance.
• Contribute to the development of a high quality local diabetes service and participate in audits/quality improvement.
