Qualifications

• Assess, plan, implement, and evaluate care pathways for people with diabetes, including newly diagnosed and long-standing cases.
• Collaborate with MDT to ensure timely, safe discharge and liaise with community diabetes teams.
• Provide advice on diabetes management and develop individualized treatment plans under supervision.
• Promote healthy lifestyles and diabetes self-management through education and practical skills.

Responsibilities

• Assess, plan, and implement care pathways for diabetes inpatients.
• Coordinate timely safe discharge with MDT and community links.
• Advise patients and carers on diabetes management and medications.
• Develop individualized care plans under the Diabetes Inpatient Specialist Nurse guidance.
• Attend MDT meetings and contribute to education and audit activities.
