The post holder will play a leading role within the Buckinghamshire Healthcare NHS Trust (BHT) Research and Innovation team, supporting our work in population healthcare and preventative medicine. This will initially be focused on the new rollout of the Trust’s innovative ‘Lipid Accelerator Programme’. This programme has been made possible by a grant award from the British Heart Foundation’s Innovation Fund.
BHT’s Research & Innovation team has an ambitious programme of work delivering population healthcare and preventative medicine at a pace and scale not previously seen. This ambitious programme is to rollout effective lipid optimisation at a population scale. We use our population-health search tool to identify thousands of patients based on their lipid levels and cardiovascular risk, before treating them with NICE-approved therapies.
This clinical role will support the appropriate triaging of patients into groups of those eligible for treatment, along with appropriate patient education. It will include history taking and clinical consultation with patients, with an expectation that the post holder is working towards a qualification as a nurse prescriber.
Main duties of the job
· Provide senior nurse management to the R&I team programmes (in particular those supporting population health and prevention), supporting programme delivery across Buckinghamshire.
· Screen and invite patients to programme clinics, according to specific protocols and guidelines.
· Either be qualified, or working towards qualification as an independent nurse prescriber.
· Request investigations according to local protocols
· Provide in-person clinics discussing therapies with patients, supporting them making decisions on specific therapies through a protocolised process.
· Provide in-person clinics delivering treatments, therapies, phlebotomy, observations and therapy discussion.
