Job summary

Lichfield and Burntwood GP Network Ltd is seeking an Independent Prescribing Clinical Pharmacist to join the team at Langton Grange Medical Centre, Lichfield. The role focuses on medicines optimisation, patient-facing clinics, and supporting pharmacy teams.

You will work within a networked GP environment, liaising with practices, hospitals, community pharmacies and care homes to deliver high-quality patient care.

Qualifications

• Masters degree in pharmacy (MPharm) and registration with the General Pharmaceutical Council.
• Holds a GPhC Independent Prescribing qualification.
• Experience and capability in primary care prescribing and medicines optimisation.

Responsibilities

• Patient facing Long-term condition clinics and review of medicines for optimisation.
• Undertake clinical medication reviews with patients and make recommendations to senior pharmacists/GPs.
• Care home medication reviews and safety improvements in medicines ordering and administration.
• Management of medicines at discharge from hospital and repeat prescribing.
• Provide medicines information to practice staff and patients and respond to queries.
