Job summary

Petroc Group Practice is seeking an experienced Clinical Pharmacist Independent Prescriber to join a progressive dispensing GP practice. You will work with GPs, Advanced Practitioners and the wider team to deliver high-quality medicines optimisation, clinical prescribing and service development in a dispensing setting.

You will conduct medication reviews, independent prescribing and medicines reconciliation, supporting complex polypharmacy and cost-effective prescribing while driving

Qualifications

• Two years' primary care/general practice experience.
• Autonomous working within your professional competence.
• Strong medicines optimisation knowledge and prescribing safety.
• Excellent communication and multidisciplinary teamwork.
• Experience with EMIS is desirable.

Responsibilities

• Undertake medication reviews and independent prescribing.
• Manage medicines reconciliation and chronic disease reviews.
• Support prescribing safety and quality improvement initiatives.
• Optimise dispensary performance and cost-effective prescribing.
• Collaborate with GPs, pharmacists and practice staff to improve patient care.
• Contribute to audits, CQC readiness and medicines management systems.
