Job summary

NHS Jobs in Slough is seeking a PCN Clinical Pharmacist (Independent Prescriber) to work with SPINE PCN GPs, resolving day-to-day medicines issues and leading repeat prescribing reviews.

The role involves medication reviews for patients with long-term conditions, supporting medicine safety, coordinating with community and hospital pharmacies, and delivering the QOF and cost-effective prescribing initiatives within the practice.

Responsibilities

• Act as point of contact in the practice for medicines related queries from health care professionals and patients.
• Review repeat prescribing reauthorisation requests and medicines nearing review dates.
• Undertake medication reviews for patients including those with multiple long-term conditions and polypharmacy.
• Support patients recently discharged from hospital to understand medication changes and ensure timely supply.
• Facilitate good relationships between the practice and community/hospital pharmacies.
• Log and action drug withdrawals and alerts to improve medicines safety.
• Assist with prescribing indicators, prescribing budget, QOF and locally commissioned services.
• Undertake minor ailment clinics or triage.
• Develop and support new working processes to optimise prescribing quality and safety.
• Attend CPPE training and development programs and participate in safeguarding and other required training.
• Participate in regular staff meetings and contribute to policy development.
