Job summary

The practice in Slough seeks an experienced Independent Prescriber to grant clinical autonomy from day one. Role includes autonomous prescribing, medication reviews, and leading on reauthorisation across multiple sites.

GPhC-registered pharmacist with 4+ years post-registration and 1+ year in Primary Care is required, along with a postgraduate diploma. NHS pension and generous leave are offered.

Qualifications

• GPhC-registered pharmacist and Independent Prescriber required.
• Minimum 4 years post-registration experience.
• Minimum 1 year in Primary Care.
• Diploma or equivalent postgraduate qualification.
• Strong medication review experience and confidence working autonomously across multiple sites.

Responsibilities

• Autonomous prescribing for patients with diagnosed and undiagnosed conditions.
• Medication reviews for patients with polypharmacy, including some home visits.
• Leading on repeat prescribing reauthorisation, post-discharge changes and safety alerts.
• Supporting prescribing indicators, QOF and quality improvement work with network impact.
• Growing Pharmacy First referrals to ease GP pressure.
