Job summary

Hilary Cottage Surgery in the South Cotswolds is seeking a part-time clinical pharmacist/independent prescriber to join the primary care team. You will lead medicines optimisation, provide expertise in safety, and undertake structured medication reviews for patients, including remote reviews and telephone consultations.

You will work alongside GPs and other healthcare professionals to improve health outcomes, support long-term condition management, and promote safe, effective use of medicines

Qualifications

• Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
• Must be a qualified independent prescriber or working towards.
• Experience with medicines optimisation and clinical reviews in primary care.

Responsibilities

• Lead on medicines use and provide expert advice to improve patient outcomes.
• Undertake structured medication reviews and telephone consultations.
• Support safe prescribing and monitoring for long-term conditions.
• Promote uptake of EPS and eRD within the practice.
• Support QOF and enhanced service objectives.
