Job summary

Humankind in Barnsley is seeking an experienced Nurse or Pharmacist with an Independent Prescriber qualification to deliver evidence‑based treatments in Substance Misuse. You will join a MDT in a dynamic healthcare team, autonomously assessing and managing patient care, and collaborating with GPs and hospital staff.

The role requires strong pharmacology knowledge, clinical prescribing skills, and commitment to CPD and governance.

Qualifications

• Essential: First level registered nurse or registered pharmacist.
• RCGP Qualifications Level 1 (and 2 or willingness to work towards).
• Proficient in Microsoft Outlook Office programmes.
• Experience of entering complex information onto an electronic/clinical data base.
• Experience in delivering evidence based treatments in Substance Misuse.
• Awareness of social context of drug misuse and the wider implications.

Responsibilities

• To deliver drug and alcohol clinical and prescribing services to the service population.
• Clinical assessment with or without Prescribing a range of medications to support recovery in line with guidelines.
• Prescribe cost effectively and advise to Pharmacists and hospital staff.
• Undertake service user centred assessments, care planning, monitoring and reviews.
• BBV management including risk assessment and testing guidance.
• Deliver health promotion and harm reduction interventions in a client-centred manner.
• Promote immunisation against Hepatitis B in line with guidance.
• Maintain clinical data databases and generate reports with data security in mind.
• Collaborate with pharmacists regarding dispensing and prescription queries.
• Share care with GPs for service users in primary care extended services.
• Deliver interventions across hubs and outreach as required.
• Engage in continuing professional development and clinical supervision.
• Adhere to governance policies and participate in clinical audits.
• Report serious incidents and disseminate learning across the service.
• Support other Humankind substance misuse services during mobilisation.
• Contribute to policy review and national/local research as appropriate.

Job responsibilities

To deliver drug and alcohol clinical and prescribing services to the service population.

Key accountabilities:

• Clinical assessment with or without Prescribing a range of medications (including controlled drugs) to support recovery in line with national and organisational guidelines and policies.
• Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital staff.
• Undertake Service User centred assessments, care planning, monitoring, evaluation and reviews in practice and outreach settings.
• Blood Borne Virus (BBV) management (risk assessment, pre and post-test counselling, testing and immunisation).
• Deliver health promotion and harm reduction interventions in a client-centred manner (e.g. safer sex, contraception, nutrition and healthy living).
• Promote and deliver immunisation against Hepatitis B in line with national guidance.
• Contribute to the maintenance of client clinical database; co-ordinate data entry and retrieval searches and reports to meet the internal and external service requirements as appropriate and ensuring data security at all times.
• Liaise with pharmacists regarding supervised dispensing and prescription queries.
• Share care with GPs for Service Users in Primary Care Extended Services (PCES)/Primary Care Recovery Services (PCRS).
• Deliver clinical interventions across hubs in different locations and deliver outreach as required.
• Undertake continuing professional development including participating in clinical supervision, performance appraisals and attending training as/when required.
• Work in accordance with all relevant legislation, best practice policies & procedures and guidelines both internal and external. This includes Humankinds clinical governance policy framework.
• Participate in the audit process and promote service wide or local clinical audit Implement findings/changes where indicated.
• Ensure all serious incidents (including deaths in service) are reported, implementing any service changes arising.
• Disseminate learning across the service and the organisation as appropriate.
• Provide support and assistance to other Humankind substance misuse services during mobilisation
• Collaborate on clinical governance, review and implementation of policies and guidelines.
• Assist in and have continued involvement with national and local research projects, as appropriate
