Job description

Be a key member of a multi-disciplinary, multi-agency team providing support to adults of all ages who do not require specialist acute hospital admission but need support to maintain their independence within their home setting. The role emphasizes promoting and maintaining individuals' independence, dignity, and choice, through medical, nursing, therapy-related treatment programs, or social care interventions.

Working across Calderdale, this role involves assessing various situations, providing solutions such as equipment, adaptations, and assistive technology, and supporting individuals. Participate in new initiatives and future changes in care delivery, including the Rapid Reablement Service as part of the Urgent Community Response Team. Support hospital discharges and prevent unnecessary admissions.

The UCR team delivers a seven-day service with staff working on a rota basis across Calderdale. The service focuses on rapid assessment and intervention for adults of all ages needing crisis intervention within a community setting due to changes in physical or psychological well-being or social circumstances requiring short-term rehabilitation or intervention.

Key Responsibilities:

1. Establish positive relationships with service users within professional boundaries, promoting dignity, choice, and respect.
2. Encourage independence by supporting personal care tasks such as washing, dressing, toileting, shaving, mouth care, catheter care, feeding, and meal preparation.
3. Support and enable mobility through exercises and use of mobility aids or equipment.
4. Accurately record and respond to support plans, risk assessments, manual handling assessments, medication profiles, and other documentation, utilizing IT systems.
5. Assist with managing prescribed medication, including dispensing, applying patches, and liaising with chemists, and support stoma and catheter care, recognizing risks and reporting concerns.
6. Assess needs for equipment, assistive technology, and adaptations; demonstrate and instruct individuals, carers, and professionals in their safe use; provide ongoing training.
7. Implement treatment protocols within competencies, monitor progress, and assist with complex treatments under supervision.
8. Support community participation by helping individuals and families identify activities to maintain social connections.
9. Use technology and interpret data to inform Reablement plans.
10. Attend training aligned with national standards, participate in induction of new colleagues, and foster positive team relationships.
11. Support and participate in treatment/exercise regimes and rehabilitation plans following appropriate training.
12. Maintain a responsible, creative, and proactive attitude, working independently and raising issues when necessary. Work in clients' homes, which may vary in condition, and escalate difficult situations appropriately.
