Description
We are seeking a compassionate and detail-oriented Independent Living Assessor to join our dedicated team at Band 2. As an Independent Living Assessor, you will play a crucial role in supporting individuals to maintain their independence and improve their quality of life. Your primary responsibility will be to conduct assessments for individuals with varying needs, ensuring that they receive the appropriate services and assistance tailored to their specific situations. This position requires a keen understanding of person-centered approaches, as well as the ability to effectively communicate with clients and their families. In this role, you will work collaboratively with multidisciplinary teams to develop and review care plans, ensuring that services align with the needs and preferences of each individual. You will be responsible for providing ongoing support and monitoring the effectiveness of interventions, making necessary adjustments to care plans as needed.
