Liverpool City Council is recruiting an Independence and Reablement Practitioner to help adults regain independence and confidence. You will work closely with a multidisciplinary team to develop person-centred goal plans and deliver focused reablement support, with progression opportunities as you complete training.

The role offers 25 hours per week, pro rata salary and a structured training programme, with potential to advance to Grade 5 after successful progression.

Responsibilities

• Independence and Reablement Practitioners support people to achieve independence and regain skills.
• Work with adults 18+ to develop person-centred goal plans and provide holistic support.
• Undertake reviews to assess outcomes and determine further care needs.
