Middlesbrough Council is seeking an Outreach Practitioner to support inclusion and identify additional needs for children and young people in school settings. You will work with a dedicated Inclusion team and deliver targeted interventions to improve outcomes.

The role operates at 32.5 hours per week during term time, with a hybrid pattern across home, office and schools in Middlesbrough. You’ll travel to settings as required, collaborating with families, staff and partner agencies.

Qualifications

• Commitment to working with children with additional needs.
• Understanding of inclusive learning and wellbeing strategies.
• Ability to support schools, families and staff with behaviour guidance.
• Experience or knowledge of supporting children with special educational needs.

Responsabilités

• Operate as part of an Inclusion team delivering high-quality interventions.
• Work in school settings on a one-to-one basis with students.
• Deliver bespoke intervention packages and attend multi-disciplinary meetings.
• Collaborate with children, families, staff and other agencies.
