Oxleas NHS Foundation Trust is recruiting a Primary Mental Health EDiC Health Practitioner to provide initial screening and assessments for new intakes in the prison estate. You will identify healthcare needs, create care plans, and refer to MDTs to ensure continuity of care during the first days in custody.

You will conduct reception screenings, provide mental health engagement, and participate in pre-release planning with the EDIC team.

Competenze

• Minimum 2 years post NMC registration as RMN/RLD.
• Essential RMN/RLD experience; prison or mental health setting preferred.
• UK registration required.

Mansioni

• Assess new intakes and identify healthcare needs.
• Make referrals to Integrated MDT and plan care.
• Support first 14 days in prison with health services.
• Contribute to pre-release planning and EDIC interventions.
• Engage in Primary Mental Health care planning and caseload reviews.
