This 12-month, innovative GP Fellowship is hosted within the South Pembrokeshire Cluster — a dynamic group of practices, community healthcare professionals, and allied partners. It is designed to support professional development through a blended role involving clinical practice, same day emergency care, hospital at home, and dedicated project time focused on improving care for people living with frailty.
