Job description

This post is a crucial development for delivering a fully integrated discharge service across Pennine Lancashire. The Integrated Discharge Service therapist undertakes highly skilled work to assess and treat patients, facilitate complex discharges, and improve clinical flow throughout the trust to enhance access to acute services.

The post holder will provide proactive and efficient Complex Case Management for all adult patients across all ELHT hospital sites. They will use specialist therapy skills to conduct assessments and case management within the integrated discharge service. Additionally, they will offer education and advice to uphold high standards in discharge planning processes, ensuring timely access to health and social care resources for safe and effective patient discharges.

The role requires flexibility to work across all trust sites and the geographical area served by East Lancashire Hospitals Trust. The post may involve working extended hours over seven days a week to support the ‘Discharge to Assess’ model of care.

Key responsibilities include:

  1. Working across all ELHT sites to ensure an efficient Complex Case Management Service.
  2. Advising and assisting staff through the discharge planning process to meet future care needs and facilitate timely discharges.
  3. Acting as a resource for ward staff and multi-disciplinary professionals, ensuring consistent and informed discharge planning.
  4. Promoting effective communication between health and social care departments in acute and community settings.
  5. Supporting early discharge planning for patients with complex needs, ensuring timely assessments.
  6. Maintaining accurate records and participating in audits related to discharge planning.
  7. Ensuring compliance with Trust policies and legislation concerning discharge processes.
