Job summary

The Fedcap Group is seeking a Navigator in Reading with hybrid working options to support participants on their journey toward improved wellbeing and employment. You will act as the trusted point of contact, guiding individuals through assessments, planning and connections to health, training and job opportunities.

With experience in health and wellbeing, disability support or VCSE environments, you will use person-centered coaching to help people overcome barriers, build confidence, and achieve

Qualifications

• Experience within supported employment, IPS, disability employment, health and wellbeing, social care, coaching or VCSE environments.
• Ability to build trusted relationships and support people with complex barriers.
• Experience using person-centered, trauma-informed, health-informed approaches.

Responsibilities

• Act as the single, consistent point of contact for participants, building trusted relationships and progress toward goals.
• Receive referrals through a warm handover and ensure participants feel welcomed and supported.
• Complete holistic, person-centred assessments of health, wellbeing, confidence and employment aspirations.
• Develop personalised support plans reflecting participants’ needs, strengths and goals.
• Provide health-informed behaviour change coaching to build motivation and readiness for employment.
• Help participants identify barriers and develop practical strategies to improve confidence and stability.
• Connect participants with community services, health and wellbeing provision, training and employment opportunities.
• Coordinate support across health, community and employment partners as required.
• Support participants to explore volunteering, training and work experience opportunities.
