Job summary

Knowsley Metropolitan Borough Council is inviting applications for a Portage Practitioner within the Knowsley Early Years Service. You will deliver an Educational Home Visiting service for pre-school children with additional needs and/or disabilities, working with parents and other agencies using the Portage Model.

You will initiate Early Help Assessments, act as Lead Practitioner when appropriate, contribute to Education, Health and Care Plans, and support regular reviews with the SEND Senior

Qualifications

• Childcare qualification equivalent to NVQ level 3 and above.
• Good understanding of Portage approach and early years support.

Responsibilities

• Initiate Early Help Assessments and act as Lead Practitioner when appropriate.
• Contribute to Education, Health and Care Plans.
• Assist the Early Years SEND Senior Practitioner with regular reviews.
• Deliver Portage services and groups for children on the waiting list.
• Liaise with Local Authority, Health, Voluntary Sector and other stakeholders to connect parents and practitioners.
