Salary: Starting salary range dependent on experience £24,500- £25,500 WTE

An exciting opportunity has arisen for a care coordinator to join an already established, growing, and committed team at Beaconsfield, providing additional care and services across our PCN patient populations, including support and enhanced care to vulnerable patients and those in residential and nursing home settings.

Main duties of the job

You will be joining a thriving existing PCN team of care coordinators, social prescribers, and health and wellbeing coaches, playing an integral role in delivering the best patient care. You will work within a dynamic, forward-thinking multidisciplinary team spanning wider PCNs, Community Services, and Local Authority, providing enhanced care to these groups.

We seek a compassionate, collaborative, and motivated coordinator to support care delivery to vulnerable patients and care homes, coordinating healthcare professionals and non-clinical staff involved in patient care.

The successful candidate will possess strong administrative skills, the ability to multitask, and excellent communication skills to liaise effectively with patients and external agencies. Sound IT skills, including Microsoft Office, Excel, PowerPoint, and Microsoft Teams, with the ability to analyze and report data, are essential.

As often the first point of contact for patients, you must communicate clearly, listen effectively, demonstrate empathy, and put patients at ease. Exceptional organizational skills and the ability to work under pressure are vital.

Job responsibilities

Primary Duties and Areas of Responsibility

Coordinate appointment bookings across various areas to ensure a positive patient experience, working with multiple team members and utilizing population health data.

Act as a contact point, advocate, and resource for patients, care teams, families, and community resources, responding empathetically and signposting appropriately.

Support patients in using decision aids for shared decision-making.

Maintain patient confidentiality and respect rights regarding sensitive issues.

Develop personalized care and support plans that address all identified needs, based on what matters most to the individual.

Assist patients in managing their needs by answering queries, scheduling appointments, and providing clear information to support informed choices.

Help patients understand their health and wellbeing activation levels, including through the Patient Activation Measure.

Identify high-risk patients and maintain a workload register for the team.

Arrange visits or appointments for patients at their practice, following clinical needs assessment, and act within the PCN's clinical skill set.

Support access to personal health budgets where appropriate.

Coordinate and navigate care across health and social care services, working closely with social prescribing link workers, health and wellbeing coaches, and primary care professionals.

Refer patients to team members or other services as needed and support effective communication within the practice network.
