Job summary

Tripod Partners seeks a Qualified Social Worker to join a hospital team at Norfolk and Norwich University Hospital. The role focuses on safe and timely discharge planning and coordinating care to maintain independence for adults during inpatient stays.

Working within a multi-agency framework, you will apply Care Act principles, conduct Mental Capacity Assessments, and engage with patients, families and carers to support recovery in a fast-paced hospital environment.

Qualifications

• Social Work qualification and SWE registration.
• Understanding of the Care Act 2014.
• Experience completing Mental Capacity Assessments.
• Understanding of Best Interests decision-making.
• Experience with Continuing Healthcare (CHC) reviews.
• Strong communication and organisational skills.
• Ability to work effectively in a fast-paced hospital environment.
