Job summary

Qualifications

• Registered with GPhC and NVQ Level 3 or equivalent required.
• ACT qualification desirable.
• Band 4–5 progression depending on experience and registration status.

Responsibilities

• Coordinate all aspects of the patient's medicines related journey with the MDT.
• Manage patient flow, complete drug histories, and counsel patients.
• Assist with transfers of care and discharges using near-patient dispensing facilities.
