Formación

• Experience in hospital/pharmacy setting preferred.
• Newly qualified technicians welcome to apply.
• Progression to Band 5 requires successful completion of in-house progression policy.

Responsabilidades

• Complete detailed medication histories from multiple sources and reconcile with inpatient medications.
• Counsel patients on new or changed medications to promote adherence.
• Assist doctors and nurses with medication queries and decision support.
• Evaluate the suitability of patients’ own meds for inpatient use.
• Participate in discharge planning with ward staff and other healthcare professionals.
• Perform accuracy checks of dispensed items in the main hospital dispensary or satellite pharmacy.
• Carry out audits to ensure department procedures are followed.
