Qualifications

• Detailed medication histories and reconciliation with inpatient meds.
• Counselling patients on new or changed medications to promote compliance.
• Support Doctors and nurses with medication queries.
• Participate in discharge planning and medicines management.

Responsibilities

• Complete medication histories from multiple sources and reconcile with current inpatient medications.
• Counsel patients on new or modified medicines to improve adherence.
• Assist clinical staff with medication queries and information needs.
• Assess patients' suitability for in-hospital use of medications and contribute to audits.
• Contribute to ward-based teaching and multidisciplinary team improvement.
