Adult Social Care in Westminster City Council is a world of extraordinary stories, where our passionate, caring and committed professionals do brilliant work every day.
Mrs D was frail and unwell. Her son was her main carer, but he wasn’t coping. We stepped in to help, but they were hostile to outside agencies. They’d always lived together and were terrified they’d be split up.
Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.
As a Hospital Discharge Support Worker you can make your own powerful contribution to Westminster’s success. Liaising mainly with teams at St Mary’s Hospital and Chelsea and Westminster Hospital, you’ll work alongside social workers and support patients as you focus on resolving any issues that might be holding up a patient’s discharge from hospital. These could include collecting clothes, completing necessary paperwork and housing applications, getting keys cut, arranging home cleaning and repairs, and providing access to other services relating to the discharge.
You’ll support informal carers with practical tasks, liaise with handyperson services relating to the installation of specialist equipment and minor adaptions, and visit patients’ homes to undertake any necessary tasks before their discharge. Other important tasks will involve making sure pets are appropriately housed if they cannot be cared for, resolving problems related to utility suppliers, and ensuring that patients’ property and personal information is handled safely and securely.
We’ll expect you to notify social workers or managers immediately if you have any serious concerns or issues affecting the patients you work with, and produce and maintain all the necessary documentation. You’ll also input data into client information systems, attend feedback sessions, contribute knowledge gained to team meetings, and be ready to report all risks, hazards and incidents promptly.

With well developed experience of working with vulnerable clients and patients with complex support needs, you’ll be a calm and collaborative team worker and possess the skills to prioritise and manage your own workload. Able to deal sensitively with patients and other service users, you’ll also have the ability to obtain agreement on proposed actions.
Your excellent communication skills will allow you to work successfully with a wide range of people and form effective relationships while maintaining professional boundaries. We’ll also expect you to be able to take onboard constructive feedback and use this to adapt and refine the way you work.
You’ll be using databases and user case management systems, so as well as good IT skills, you’ll also be ready to negotiate with services and suppliers of practical help. In addition, you should be adept at dealing with any setbacks you encounter, and persistent when it comes to finding solutions to problems.
