As a Hospital Discharge Support Worker, you will liaise mainly with teams at St Mary's Hospital and Chelsea and Westminster Hospital, working alongside social workers to resolve issues delaying patient discharge. Tasks include collecting clothes, completing paperwork, arranging home repairs, supporting carers, and ensuring safe housing for pets, among others.
You will also need to notify social workers or managers of serious concerns, maintain documentation, input data into information systems, attend feedback sessions, and report risks and incidents promptly.
You should have experience working with vulnerable clients with complex needs, be calm, collaborative, and able to manage your workload. Excellent communication skills are essential, along with proficiency in IT and case management systems. You should be persistent and solution-oriented.
