  • Ensure safe and smooth discharge of patients from the hospital.
  • Collaborate with patients, families, and healthcare professionals to coordinate post-discharge services.
  • Develop and implement appropriate care plans tailored to individual patient needs.
  • Act as a liaison between hospital staff and external care providers to ensure continuity of care.
  • Address any challenges or concerns to facilitate a seamless transition for patients.
