Responsibilities
  • Conducting detailed assessments to develop safe, personalised care plans.
  • Collaborating with Occupational Therapists, support managers, and mental health teams.
  • Facilitating placements into our new reablement and step-down facility.
  • Reviewing and adjusting care plans to adapt to changing needs.
  • Maintaining accurate records in line with local policy and best practices.
