Job Description:

We are recruiting for a Qualified Social Worker to join a Hospital Discharge Team in the Greater Manchester area.

About the team

This team supports individuals and their families or carers to adjust to illness, access support, and plan for any post-discharge resources they might need. The organisation is committed to supporting employees to work with agility, where appropriate for the post and service requirements.

About you

The ideal candidate will have good post-qualifying experience in Adult Social Work. Experience within Hospital or Intermediate Care is advantageous. A degree in Social Work (Degree/DipSW/CQSW) is required. A Practice Educator qualification is preferred. You must also be registered with Social Work England.
