Full Time, Fixed Term - Maternity Cover

We are recruiting 1 full time (37 hours) Team Manager for the Hospital Discharge Service - Acute team to provide Maternity cover for up to 12 months - could it be you?

Responsibilities

As well as proven leadership skills you will need to be able to work as a strong team member, be responsive, build frameworks of support, develop best possible strengths-based practice and participate in process development and review.

Practitioners will need to be supported and resourced to manage the pressures of the role and to make quick evidence-based decisions.

The Hospital Discharge Service - Acute team comprises of 2 multi-professional practitioner groups who work within the Transfer of Care Hubs based at Southmead Hospital and Bristol Royal Infirmary in Bristol.

The practitioners work in person in both hubs daily, where they work closely with health and voluntary organisation colleagues to triage referrals to ensure that the right discharge decisions are made with people who are ready to leave hospital.

The practitioners follow a strong 'Home First' approach, using a strength based 3 tier model. The team work collaboratively with health colleagues on Safeguarding concerns for people who are in hospital or where concerns will have an impact on their discharge outcomes.

As necessary care act assessments, re-assessments & reviews are completed for a small group of people whilst in hospital, the team will identify need, apply eligibility criteria, establish appropriate services and on discharge transfer cases to our hospital discharge service community teams or longer-term locality teams for ongoing review.

You will need the ability to maintain data relating to our service area Key Performance Indicators and drive service improvements across the health and social care system to support hospital discharges in a timely way.

You will need to analyse practice, review the service and implement change as needed.

The Team Manager role is key to furthering and sustaining positive relationships with internal and external partners (e.g. acute and community health partners, BCC housing, wider ASC teams, voluntary sector organisations).

Requirements / Qualifications

Please note that this role is subject to an Enhanced DBS Check.

For permanent Bristol City Council employees, this post will be a secondment.

For employees who are currently on a fixed term contract with Bristol City Council, the secondment must not extend beyond the end of the fixed term period to ensure there is a substantive post to return to.

If you are currently a casual worker or a member of an employment agency (currently working with Bristol City Council) this post will be a fixed term contract.
