Job Description:

The post-holder will be responsible for proactively delivering housing options advice to applicants being discharged from hospital or about to be discharged. Responsibilities include ensuring security of tenure, promoting low-cost home ownership, preventing homelessness, investigating household circumstances, and undertaking statutory assessments to determine the best options and resources on a case-by-case basis. The role involves managing a caseload across housing options, move-on, and homeless prevention, supporting clients in accessing various housing options, and making relevant referrals and signposting to other services.

Salary: £37.96 per hour
