Hertfordshire Community NHS Trust is seeking an experienced Registered Nurse or HCPC Registered Paramedic to lead the Hospital at Home Service in Stevenage. The role combines advanced clinical practice with operational coordination and staff leadership across Hospital at Home, Remote Monitoring and Home Health Monitoring pathways.

Qualifications

• Registered Nurse or HCPC Registered Paramedic with significant post-registration clinical experience.
• Strong clinical assessment and decision-making skills with ability to recognise deterioration.
• Clinical leadership and management experience, coordinating services and priorities.
• Excellent communication and multidisciplinary working skills.
• Independent Non-Medical Prescriber, or willingness to undertake the qualification where appropriate.
• Experience of audit, quality improvement or service development in community/ Hospital at Home.

Responsibilities

• Triage referrals and discuss with clinicians to support admission, movement and discharge within Hospital at Home pathways.
• Undertake advanced holistic patient assessments and identify deterioration to inform management plans.
• Coordinate Hospital at Home, Remote Monitoring and Home Health Monitoring caseloads balancing acuity, capacity and staffing.
• Provide clinical and operational direction to staff and address risks accordingly.
• Participate in multidisciplinary meetings and ward rounds, virtual or in-person.
• Provide day-to-day leadership and line management to an identified team.
• Support staff supervision, wellbeing, appraisal and development.
• Lead or contribute to clinical audit, quality improvement and governance activity.
