Job summary

Central and North West London NHS Foundation Trust invites applications for a Virtual Ward Senior Practitioner in Milton Keynes. The role focuses on delivering high-level clinical assessments and care plans for acutely ill patients at home, using technology for monitoring and diagnostics.

You will work within the Virtual Ward, coordinating pathways across organisations, leading MDT discussions, and supporting discharge planning while ensuring patient-centered care.

Qualifications

• Registered Nurse/Advanced Practitioner with capability for advanced triage and assessment.
• Experience in community/virtual ward settings is desirable.
• Full UK driving license and access to a car are required.

Responsibilities

• Undertake advanced clinical assessment of patients on the Virtual Ward.
• Develop, implement and evaluate care plans for acutely ill patients at home.
• Coordinate patient pathways and liaise with MKUH and SDEC as needed.
• Oversee caseload management and supervise staff as required.
• Ensure timely discharge planning and remote monitoring of patients.
