Service Care Solutions is recruiting a Hospital Adults Social Worker for Salford Royal on a two-week contract. The role focuses on assessing adult patients' social needs, developing care plans with the clinical team, and facilitating safe discharges back into the community.

You will work office hours, Monday to Friday, 08:00 to 16:00, collaborating with multidisciplinary colleagues, maintaining records, and upholding safeguarding standards.

Qualifications

• Holds relevant social work qualifications and registration.
• Minimum 3 years’ experience in adult social care, hospital or community settings.
• Excellent communication and interpersonal skills.
• Able to work independently in a busy hospital environment.
• Understanding of safeguarding procedures and care planning.
• Available for the full 2-week contract, Mon-Fri 08:00-16:00.

Responsabilités

• Conducting thorough assessments of adult patients’ social needs.
• Developing and implementing care plans in collaboration with healthcare teams.
• Supporting patients and families through complex decision-making processes.
• Coordinating with multidisciplinary teams to ensure smooth patient discharges.
