Job description

We'relooking for an individual to assistwith the delivery of holistic care to patients, family/carers with lifelimiting illness, within their own living environment.

Contract: Permanent

Salary: £24,793-£26,444 per annum (pro rata), Band 3 Competency Based Framework.

WorkingPattern: 37.5hours per week. Weekend work required. Theservice currently operates seven days per week, between 8am and 8pm.

Based at: Little Hulton and Trafford

The successful candidate will be subject to a Standard/EnhancedDBS Check, the cost of which is covered by the hospice. Main duties of the job
  • Playa key role in the provision of specialist palliative and supportive care.
  • Involvepatients and their families first and always go the extra mile to provide firstclass care.
  • Playan integral part of the multidisciplinary team and have strong workingrelationships with other health care providers.

Whilstyou will have a specific base, there may be occasions where you are required toprovide cross-site/service cover.

Job responsibilities

The H@HHealthcare Assistant will:

  • Assistin assessment, planning, implementing and evaluating care in partnership withthe patient, family & carers.
  • Ensureaccurate record keeping in accordance with organisations standards.
  • Communicatechanges and ongoing care needs of the patient to the coordinator and DistrictNurses as appropriate.
  • Havea knowledge of local services appropriate for the patient and family/carer.
  • Ensurecare is consistent with the 5 principles of the Mental Capacity Act Code ofPractice.
  • Beable to access and support complex holistic needs of patients, families andtheir carers.
  • Providephysical, psychological and spiritual support and advice to patients and their carers.
Person Specification
Experience
  • Demonstrate previous hospital experience or experience of working in a similar care setting.
  • Demonstrate an understanding of the Hospice @ Home service.
  • Demonstrate knowledge of other community services.
  • Demonstrate an interest in specialist Palliative care.
  • Work as a team player.
  • Maintain strict confidentiality.
  • Display a kind, emphatic and person centered approach.
  • Demonstrate knowledge of clinical governance and personal responsibilities
  • Use their own initiative.
Qualifications
  • Have or be willing to work towards NVQ level 2 in Care or equivalent.
  • Be willing to undertake Compassion in care course
  • Hold a full driving license and have access to a vehicle with car insured for business use.
  • Ideally have undertaken training in Communication.
