HOPE(S) Specialist Practitioner Band 8B
Secondment – 2 years
We are pleased to offer an exciting opportunity for a highly skilled clinical practice leader to join us in the North West HOPE(S) team as a Specialist Practitioner.
Are you passionate about and committed to reducing restrictive practices? Are you looking for a role with lots of variety? Are you looking for experience of working in the wider health and social care system whilst using your clinical skills working with people in Long Term Segregation?
Due to new investment and extension of funding we have 2 x 2 year secondments available:
HOPE(S) Practitioners work directly with people who are referred to the programme and their teams, using the HOPE(S) model and clinical skills and experience to reduce restrictive practices, end or reduce time in segregation, to provide clinical input into discharge planning and to clinically support commissioners and commissioned teams and providers to avoid admission and escalation to secure services.
The HOPE(S) Clinical Model was developed by Mersey Care NHS Foundation Trust and the trust hosts this work in the North West,
Please note-the successful post holder will work in direct clinical care environments across North West England and occasionally support North West funded patients elsewhere
Main duties of the job
To provide strategic clinical leadership and innovation using the most up to date and evidence-based theory and practice with a specific focus on the application of the HOPE(S) clinical model of care to reduce long term segregation and innovative strategies to improve clinical practice and reduce restrictive practices with children and young people, adults with autism and/or a learning disability and those with mental ill-health across Integrated Care Boards (ICB’s) and Lead Provider Collaboratives in partnership with Mersey Care NHS Foundation Trust.
Mersey Care is one of the largest trusts providing physical health and mental health services in the North West, serving more than 1.4 million people across our region and are also commissioned for services that cover the North West, North Wales and the Midlands.
We offer specialist inpatient and community services that support physical and mental health and specialist inpatient mental health, learning disability, addiction and brain injury services. Mersey Care is one of only three trusts in the UK that offer high secure mental health facilities.
This secondment provides a rare opportunity to develop specialist clinical skills working into multiple health and social care providers and will provide with development opportunities which they can utilise on return to their own Trust

To provide Clinical and Practice leadership for the HOPE(S) programme. To deliver key outcomes on this portfolio to the Programme Steering Group.
To offer expert clinical advice and chair meetings with organisations within Prospect Lead Provider Collaborative.To offer expert clinical advice for service users, carers, staff and other professionals. To demonstrate advanced competence in highly complex decision making, assessment and the management of clinical needs within
In Patient Services across Prospect Lead Provider Collaborative. To work in partnership with service users, carers and other practitioners to ensure effectiveness of outcomes.
