Job summary

Mid Yorkshire Teaching NHS Trust is seeking a Clinical Pharmacist for the Homecare service based at Pinderfields Hospital, Wakefield. You will work with Advanced Clinical Pharmacists and clinical teams to deliver a high-quality, cost-effective clinical pharmacy service to Homecare patients across multiple specialties.

You will contribute to Medicines Optimisation, provide ward-level support and MIU coverage, and participate in weekend/bank holiday/on-call rotas as part of an expanding Homecare

Qualifications

• Provide a high-quality clinical pharmacy service to a defined clinical area.

Responsibilities

• Clinical validation of Homecare prescriptions across a broad range of specialties for new and existing patients.
• Plan and implement biosimilar switches in collaboration with clinical teams and stakeholders.
• Optimise cost-effective use of High-Cost Medicines across Homecare Service and the Medical Infusion Unit (MIU).
• Plan and carry out regular audits to demonstrate cost-effective use of medicines and adherence to clinical guidance.
• Produce and analyse reports on medicines spend within relevant areas.
• Support Medicines Optimisation and provide services at ward level and to MIU; contribute to weekend/bank holiday/emergency rota.
