Job description

The post holder will provide community/home environment-based support to service users with mental health needs who are in crisis, promoting their recovery. The role includes supporting the carers of service users receiving home treatment. The post involves working with clients with a wide range of mental health problems in a culturally diverse borough. The postholder will work within a multi-professional team to implement, evaluate, and deliver high-quality care and treatment to service users and carers.

  • Multi-disciplinary Team - daily
  • Service Users and Carers - daily

  • Head of Profession - as required
  • Inpatient, Liaison and Community Teams - regularly
  • GP (General Practitioners) - regularly
  • Children and Families Services - as required
  • Other agencies - as required

The postholder must be a Registered Mental Health Nurse, Social Worker, or Occupational Therapist with an established clinical background, having completed necessary academic and clinical supervision to meet competency requirements for a Home Treatment Team Practitioner. The post may require working across different Trust sites based on service needs.

The Trust's Home Treatment Teams provide 24/7 crisis assessment and intensive home treatment to prevent or facilitate earlier discharge from inpatient care. The post holder will provide care within their professional boundaries, from initial history taking, assessment, diagnosis, treatment, to evaluation of care.

Responsibilities include:
  • Contributing to ongoing assessments, including risk assessments
  • Assessing capacity and providing safe treatment and care
  • Reporting on patient condition and response to interventions
  • Operating within legal frameworks such as the Mental Health Act
  • Escalating complex long-term condition needs
  • Maintaining clinical and pharmaceutical knowledge
  • Promoting evidence-based care in patients' homes
  • Carrying out physical observations and liaising with medical staff
  • Administering medication safely and effectively
  • Supporting recovery strategies, health education, and self-care
  • Supporting carers and families
  • Applying infection control measures
  • Adhering to lone working practices and coordinating shifts
Communication and Service Development:
  • Communicating health status effectively
  • Providing accurate information with sensitivity
  • Acting as an advocate
  • Maintaining accurate records
  • Participating in service evaluations, research, and audits
