South London and Maudsley NHS Foundation Trust is seeking a Home Treatment Team Practitioner (NHS AfC: Band 6) in London. You will work within Older Adults HTTs, providing crisis resolution and home-based care to adults over 65 in the community, with supervision and development support.

You will lead care delivery, coordinate MDT input, assess risk, plan care, and maintain excellent communication with service users and carers in their own homes.

Qualifications

  • Registered nurse with appropriate registration (Adult/Mental Health/Learning Disabilities).
  • Evidence of continuing professional development is desirable.
  • Certification or training in supervision or mentorship is preferred.

Responsibilities

  • Coordinate and evaluate patient-centred care with the MDT to improve plans.
  • Provide accurate care information in accessible formats for patients and carers.

Job overview

We are looking for a Home Treatment Team Practitioners to be part of our motivated and experienced teams.
While working with the team you will gain experience in relation to risk assessment, risk management, care planning, engagement and communication with Clients who are experiencing a mental health crisis and in need of support in their own homes.

As a Home Treatment Team Practitioner working within the South London Mental Health and Community Partnership you are required to encourage and foster collaborative and cross organisational working.

Main duties of the job

Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality nursing care, contributing to the physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.
Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support:

  • Co-ordinating and evaluating collaborative patient centred care; getting feedback and improving plans with the multi-disciplinary team (MDT).
  • Providing accurate information about care in an accessible format to patients and their relatives / carers.
  • Liaising and working positively with members of the multi- disciplinary team and partner organisations.
  • Administering, and monitoring the side-effects of, medication; monitoring service user compliance.
  • Safety; safe custody of medicines, sharps, clinical equipment, report all incidents following trust policy.
  • Leading and supporting/teaching junior staff on risk assessment and risk management that involves making decisions based on knowledge of the research evidence, knowledge of the individual service user and their social context, knowledge of the service user’s own experience and clinical judgment.
Working for our organisation

The Older Adults Home Treatment Teams (HTT’s) provide comprehensive and accessible crisis resolution and home-based care and treatment to people in the acute phase of mental illness in their own homes as an alternative to hospital admission.
The primary function of the teams is to provide intensive treatment in the persons own home or other suitable environment and gatekeep referrals for older adult inpatient beds and facilitate early discharge from hospital.
The HTT’s are a multidisciplinary service providing care to residents primarily over the age of 65.

The Team is based in Maudsley Hospital, 111 Denmark Hill, London SE5 8az

Person specification
Education and Qualification
  • Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse (2)
  • Evidence of continuing professional development
  • A certificate in Practice Supervisor Training, Practice Assessor Training, Practice Supervisor or Assessor Training, SSSA-aligned training, or a recognised Mentorship Certificate (or equivalent qualification)
Experience and Knowledge
  • Experience of research-based/reflective practice, e.g. understanding and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence based practice
  • Significant relevant clinical experience
  • Evidence of continuing professional development
  • Experience as a preceptor / mentor
