Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for full-time, part-time, or PRN home health visits in the Woodbridge, VA area, serving adult and geriatric clients.

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

  • Making home visits within designated territories.
  • Performing diagnostic tests and measurements related to mobility, strength, balance, and safety.
  • Implementing wound care interventions using evidence-based dressings and topical agents.
  • Developing and adjusting individualized care plans, including manual therapy, gait training, and balance exercises.
  • Assessing and revising care plans, collaborating with nursing staff.
  • Educating clients and families on rehabilitative activities to promote health and independence.
  • Documenting care activities accurately using electronic tablets.

Qualifications:

  • Current VA Physical Therapist license.
  • Graduation from an accredited program by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficiency with computers; experience with electronic medical records preferred.
