Job description

The Home First service brings together nurses, therapists, health and therapy assistants, to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient's own home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission. Home First will, where required, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

You could become part of our exciting service, bringing the unique benefits of a wraparound recovery service from hospital discharge. The principle of the service ensuring that recovery involves working towards the person being as independent as possible through enablement.

We are looking for a Senior Physiotherapist to join us, so if you're an ambitious and proactive physiotherapist looking to join a team that provides cutting-edge, excellent community care, we'd love to hear from you!

Main duties of the job

a) To work effectively as a team member in providing and facilitating a high standard of care, social and rehabilitative support in the unit, department or in the community.

b) To plan therapy, social and rehabilitative pathways taking account of the fluctuating needs of the patient and assist in evaluating the outcomes.

c) To report and document variances in a patient's condition or planned pathway of care.

d) To offer advice and support and education on healthcare to both patients and carers.

e) To take referrals for all services using the criteria specified in their operational policies.

f) To undertake a holistic assessment of all the patients' needs and understand the routes for escalating issues that are not directly resolved by own professional practice.

g) Formulate and deliver an individual treatment programme based on a sound knowledge of evidence-based practice and treatment options using specific clinical assessment, reasoning skills and knowledge of treatment skills.

h) To evaluate patient progress, reassess and alter treatment programmes as required.

i) Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

j) To manage clinical risk within own patient case load and recognise potential risks to others, complying at all times with the requirements of health and safety regulations.

Person Specification
Qualifications
  • Professional qualification such as a degree or equivalent in Physiotherapy.
  • Current registration with the Health and Care Professions Council
  • Evidence of relevant postgraduate courses / CPD and reflective practice relevant to clinical speciality
  • Training in student education
Experience
  • Substantial relevant post-qualification experience covering a broad range
  • Experience of working within a relevant specialist area
  • Experience of interdisciplinary working
  • Experience of supervising junior staff, assistants and students
  • Experience of independently managing a clinical caseload
  • Experience of contributing to audit and evaluation within a clinical governance framework
Skills and Knowledge
  • Able to demonstrate competency in a range of clinical skills
  • Able to assess, plan, implement and evaluate care for individuals
  • Able to plan and prioritise own workload and delegate appropriately
  • Able to demonstrate leadership in practice and supervise junior staff
  • Up-to-date knowledge of clinical governance, audit, risk management, and professional codes
  • Evidence of CPD and commitment to ongoing learning and development
