A large healthcare provider is seeking a dedicated practitioner to support patients managing feeding tubes in their homes. This role involves home visits, data management, and collaboration with healthcare teams to ensure smooth transitions for patients. Applicants should have experience in health or social care, excellent communication skills, and a passion for improving patient care. Flexible working hours are available, ideally 3 days per week.
University Hospital Southampton NHS Foundation Trust is delighted to offer a fantastic opportunity to work with us. Please see below for detailed job description of the role.
Would you like you to help people keep well in their own home? Are you interested in supporting people who receive a tube feed? We are looking for an enthusiastic practitioner to support our patients as they are discharged to their home environment with a feeding tube and then helping support them once they are at home. This is an established, very rewarding role which would suit someone with previous experience in a health or social care setting who would like to gain more expertise in this field. You will be working across 2 community home enteral feeding teams. You will work for 2 days per week in the Southampton City Home Enteral Nutrition Service and one day per week in West Hampshire Enteral Nutrition service. This role is vital in supporting patients to manage their tube feeds in their own homes. Both teams work very closely together and have excellent links with dietitians and nutrition nurses in Southampton General Hospital.
