Job description

We are looking for enthusiastic and motivated support workers to join our Integrated Urgent Care Team within our community team. The Integrated Urgent Care Team provides short-term interventions to maximize independence and responds to urgent/crisis situations to enable people to continue living in their own home for as long as possible. The team responds to patients discharged from hospital wards, A and E, and MAU (Medical Assessment Unit), assists in preventing inappropriate admissions to hospital, as well as supports those in crisis in the community.


The team is made up of staff from health and social care, working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible. The team provides support to younger and older adults with varying degrees of disability, social complexities, and vulnerability living in the community.


Shifts are carried out over a seven-day rota including weekends and bank holidays; shift patterns are 07:30 - 15:30 or 14:00 - 22:00.


Please note that this service is based in the community, and travel will be an essential part of the role. Prospective applicants will be expected to travel across Tameside and Glossop.


As a key member of this busy but supportive team, you will be required to work in a practical and positive way to enable service users to regain confidence and to motivate them in regaining daily living skills.


This post is subject to a rota pattern working over 7 days a week and includes some weekend and bank holiday working. It is essential that you hold a current valid driving license and have access to a vehicle for work use, for which a casual car allowance will be paid.
