About the Role

As theHome Care Manager, you’ll be responsible for leading and overseeing daily operations, ensuring top-tier service delivery, and supporting your care team to deliver person-centred care that promotes independence and quality of life.Key Responsibilities:

• Leading and mentoring a team of care workers
• Developing and reviewing client-centred care plans
• Ensuring compliance with aged care standards and regulations
• Conducting regular client assessments and service reviews
• Liaising with healthcare professionals, clients, and families
• Managing care budgets and service agreements
• Promoting continuous improvement and quality assurance

What We’re Looking For

• Registered Nurse (RN) or Enrolled Nurse (EN) with current registration
• Proven leadership and organisational skills
• Excellent interpersonal and communication abilities
• Strong knowledge of aged care regulations and home care best practice
• Proficiency in using electronic health records or care management systems
