Job summary

NELFT is seeking a Physiotherapist to deliver specialist community stroke and neuro assessments in patients’ homes, develop and adjust treatment plans, and provide equipment as needed. You will work within a multidisciplinary team across Trust sites, supervising rehab assistants and deputising for senior colleagues as required.

The role supports pre-registration students, delivers direct patient care, maintains records, and participates in service improvement initiatives.

Responsibilities

• Specialist community stroke and neuro assessment in homes
• Establish and evaluate appropriate treatment plans
• Equipment provision
• Work as part of a Multidisciplinary Team
• Supervision of Band 3 & 4 Rehabilitation Assistants
• Deputising for the Band 7 Team Leads
• To support Pre registration students
• Deliver day-to-day patient care, administrative support, data entry.
• Liaise with internal and external stakeholders to support service delivery.
• Maintain accurate records and ensure compliance with Trust policies.
• Participate in team meetings and contribute to service improvement initiatives.
