A healthcare provider in Barnsley seeks an experienced HIV Nurse to deliver care for patients living with HIV as part of a multi-disciplinary team. Responsibilities include patient support, care coordination, and effective communication. Applicants should be registered adult nurses with a passion for HIV nursing care. This part-time role offers a salary of £29,970 to £36,483 annually, with opportunities for professional development and a commitment to inclusive employment.
The post holder will practice as part of a multi-disciplinary team to provide high quality care for people living with HIV (PLWHIV) under the care of Barnsley HIV Service. This care will be delivered as directed by the HIV medical team.
The post holder will:
Offer support and care coordination to clients, their relatives, partners and carers.
Undertake patient related care under the direction of the HIV medical team.
Work with the wider MDT to manage lost to follow-up patients and deliver a model that allows safe monitoring and management of patients to allow them to safely re-access care within Barnsley HIV Clinic Services.
Provide regular reporting on service activity as required and requested.
Participate in audit.
Attend regular clinical review meetings of complex patients.
The post holder might be required to work across Barnsley and local areas at any time throughout the duration of their contract. This may entail travel and working at different hospital sites for meetings or occasional patient care.
The post holder will support the multi-disciplinary team in the provision of an effective and safe service for the assessment, treatment, monitoring and education of people living with HIV.
This nursing care will be evidenced based and take into consideration the lifestyle, gender, sexual orientation and cultural background of the client group.
