Job Description:

We are looking for a self-motivated, organized individual passionate about patient care and excited to independently manage cases with confidence and accountability. As a Home Hemodialysis Nurse (HHD Nurse), you will educate patients and caregivers about Home Hemodialysis in a clinical setting, enabling them to dialyze at home. You will oversee ongoing patient care to ensure treatment adherence and success.

What you can expect:
  1. Provide comprehensive patient care independently.
  2. Assess, diagnose, and implement care plans, managing your own patient caseload.
  3. Empower patients through education on administering dialysis and managing their health.
  4. Build strong relationships with patients, support systems, and care partners.
Schedule:

Typically an 8-hour day, Monday through Friday, with weekends and holidays off. Flexibility to manage your own cases and schedule.

What we'll provide:

  • Paid 9-12 week training for those new to dialysis, with support to become a nephrology nurse.

Requirements:
  • Valid driver's license, reliable transportation, and vehicle insurance.
  • Associate Degree in Nursing (A.D.N.) from an accredited school; B.S.N. or diploma may substitute.
  • Minimum of 5 years RN experience.
  • Basic computer skills (MS Word, Outlook).
Highly Preferred:
  • Experience with Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis, or HHD.
  • Background in training, teaching, or healthcare education.
  • Case management experience.
  • Interest in telehealth technologies.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
